You must notify CRI of any changes that would affect your coverage. This includes the addition of new family members (e.g., birth, adoption, marriage) or covered dependents no longer meeting the plan's eligibility requirements (e.g., divorce).

MAKING CHANGES DURING THE YEAR

Your enrollment elections for medical/vision, dental, and the Health Care or Dependent Care Reimbursement Accounts (HCRA/DCRA) are irrevocable until the next annual open enrollment period. However, changes are permitted if you have a qualifying event and notify CRI's Operations Manager within 31 days of the event. Failure to notify the company may result in your having to pay for coverage for family members who are no longer eligible.
